Living Healthy with Chronic Pain

6 week workshop

A free Chronic Pain Self-Management workshop for any individuals living with chronic pain (such as musculoskeletal pain, fibromyalgia, repetitive strain injury, chronic regional pain sydrome) and their caregivers. Small groups of participants (usually 10 to 16) meet for 2.5 hours, once a week for six weeks. The highly interactive sessions are led by pairs of trained program leaders.

Living Healthy with Diabetes

A free Diabetes  Self-Management workshop for any individuals living with diabetes and their caregivers. Small groups of participants (usually 10 to 16) meet for 2.5 hours, once a week for six weeks. The highly interactive sessions are led by pairs of trained program leaders.

Help people make concrete action plans that address the aspects of their health or situation that are most important to them.

Brief Action Planning (BAP) is a self-management support tool based on the principles and practices of Motivational Interviewing. The Brief Action Planning workshop provides health care providers with three highly structured, patient-centered, evidence informed self-management support tools grounded in the principles and practice of Motivational Interviewing: Give No Advice, Ask-Tell-Ask, and Brief Action Planning.
