Monthly Peer Support Group

The purpose of the Reconnect & Refresh is:

• to gather former participants to meet and reflect on their 6-week program, 
• to give feedback on the effect that their self-management program had on them, 
• to refresh the concepts that they discovered there and,
•  to reconnect with the program and the people they met.

Occassionally a guest speaker with expertise on chronic illness management will also provide a presentation during the session. Tools and resources will be provided to participants at no cost!
